Welcome to Banner Alzheimer’s Institute. Our entire team is committed to ensuring that you have a positive experience, beginning with your first contact to schedule an appointment at either our Stead Family Memory Center in Phoenix, Arizona or our Cleo Roberts Memory Center in Sun City, Arizona.

Our cognitive assessments are comprehensive and – depending on your insurance coverage – can require multiple steps. That’s why we’ve created the following three-part process:

1 – Gather information

We begin by collecting information from you. Please complete the entire New Patient Packet, sign the form and send it to either our Phoenix or Sun City location by secure fax or mail:

2 – Verify coverage

After we receive your completed New Patient Packet, the next step is to verify your insurance coverage. Depending on your insurance, this can take several weeks so we appreciate your patience. Once your insurance carrier confirms your coverage, we’ll contact you to schedule an appointment.

3 – Obtain authorization

Frequently, insurance plans also require authorizations to perform scans, lab work and other tests, including:

  • Magnetic Resonance Imaging (MRI)

  • Positron Emission Tomography (PET)

  • Neuropsychological examinations

  • Other tests

We’ll complete the authorization process as quickly as possible, then contact you to schedule the tests.

Need to reschedule an appointment?

  • New patient or return appointments—Please contact us one or two business days before your appointment, if you need to reschedule or cancel. 

  • Neuropsychological tests—Please contact us three business days prior to your appointment, if you need to reschedule. If three days advance notice is not provided, a cancellation fee may be applied to your account.

Returning telephone calls

Assisting our patients and their caregivers is a top priority. Our Memory Center team members make every attempt to answer phone calls during regular business hours. If you leave a voicemail, we’ll return your call within 24-48 hours. 

Requesting medical records

All requests for the release of medical records must be submitted in writing. We require five days to collect the records. We also require that the requestor sign a medical records release (HIPAA) form. This ensures the organization or individual requesting the records has the authority to obtain them.

Obtaining prescription refills

We ask three business days’ notice for prescription refills. Please call your pharmacy to request the refill. Your pharmacy will contact us via fax to submit the refill and we’ll respond with a prescription, along with instructions, within three business days.
